Minor quest bug: during Stranded at the marsh you are supposed to rescue 5 villagers but instead, my objectives say to kill 5 Crocolisks, which is what you are required to do to rescue them but the display is supposed to say to rescue the crash survivors. also some of the Crocs aren't skinnable.

The quest Take Back What's Ours is missing a part so to speak. When approaching the cabin, you encounter several Veteran Dark Ranger, these are missing, when using the Horn of Tal'doren Tobias Mistmantle and several Tal'doren Trackers are supposed to appear and help you, using the horn does nothing though and Tobias Mistmantle is already spawned alone outside the cabin.

Betrayal at Tempest's Reach as a missing scene and after completing this one the watchmen are supposed to become friendly which never happens for me.
Also after accepting Flank the Forsaken Lord Hewell is supposed to be able to give you a horse, but is unclickable.

Liberation Day just auto completes. You should have to Free 5 Enslaved Villagers inside the Emberstone Mine by freeing them from the ball and chain.

The npc's in Emberstone Village are all lying on the ground, you can still kill them though so the quest Push them out is completable.

The Battle for Gilneas City is completly bugged, except beeing able to use the sword to inspire the citizens.
The speech is completly missing. The Boss Gorerot is also missing.

Note: This ends what I was able to recover from google cache and my browser cache. After this I only have the list of the sites I visited, so all reports that are missing after this one will be harder to restore and perhaps sometimes not quite acurate.

This is from what I recall posting not 100% sure that it's accurate:

The Hunt For Sylvanas just auto completes itself. Tobias Mistmantle just stands there instead of going out to search for her.

Slowing the Inevitable also just auto completes. The Bat is there but it acts as a Transmog Npc!?.

Knee-Deep as a minor bug, using the torch does not make the criters runaway. although the fear symbol does show up above their heads.

After completing Patriarch's Blessing which is missing the cutscene the undead are supposed to disappear as seen here.

They Have Allies, But So Do We is as far as I recall also auto completed. Boarding the Glaive doesn't work.

Endgame also just auto completes.

The Howling Oak can't be obtained because Krennan Aranas is missing from Rut'theran Village (which looks rather empty by the way, not sure if bugged)

In Darnassus there are several missing npc's:
Genn Greymane, Taladan, Feyden Darkin, Tana Lentner, Hugo Lentner, Foreman Pernic, Rolf Karner, Mythrin'dir, Rissa Halding, Cyridan, Saenorion, Eladriel, Fyldan, Me'lynn, Telonis, Tarien Silverdew, Innkeeper Saelienne.
Not 100% accurate since I based this on my browser history.

Layna Karner is spawned at the wrong location and as a wrong display id.
Gwen Armstead is supposed to be an Innkeeper.
The Frostsaber Companion npc seems to have an Hammer equiped.

Missing: Anadyia, Vinasia, Turian, Mythidan, Aryell Skyshadow.

Breaking Waves of Change has a bug. You should go to Vesprystus, and get a ride to Lor'danel, but the end location can't be chosen unless you have been there and spoken to the Hippogryph Master there.

Dentaria Silverglade and Serendia Oakwhisper are invisible to non gm's.

The last wave of survivors is missing all objective npc's:
Cerellean Whiteclaw
Gershala Nightwhisper
Shaldyn
Volcor.

Ranger Glynda Nal'shea is invisible too.
Fixed in V4

Hammon the Jaded is missing from Darnassus.
Fixed in V4

I believe this is everything that was lost. From this post on i'll be posting what I find new.

No Accounting for Taste has the objective bugged, the bear carcass is lying arround but not interactable with.

Bearer of Good Fortune not working, you can open the cages with the key for a second but neither do the bears come out nor does it count.

Tharnariun Treetender should be spawned near the cave of the above quest.
Fixed in V4.

A Troubling Prescription isn't obtainable although the yellow ! is floating above the table there is also a parrot?! and neither are clickable.

The quest:

A lost companion can't be obtained, Volcor is missing (most certainly due to the bug with the quest The last wave of survivors). Even after spawning him, he doesn't offer the quest.
After adding the quest manually, it just auto completes it self. This video also shows the rest of the npc's missing at that location.

A love eternal is also missing it's quest giver Cerellean. Spawning him allows you to get the quest though.

Power Over the Tides isn't obtainable. After adding it manually, you still can't complete it. Using the orb of elune on the remains works and the tranquil spirit shows up but it doesn't count towards the objective, unless you kill the tranquil spirits with .die command.

The Moonstalkers in Darkshore are neutral and should be hostile. Furthermore I can't obtain the Moonstalker Whiskers from them, making the quest Ritual Materials impossible to complete. Actually none of the items required for the quest are obtainable.

The creature Shadowclaw is spawned at least twice with different models. One of them seems to be holding a staff!?

Twice Removed is partially buged. Killing Lady Janira works fine, also calling the withered ents is working but they won't count towards the objective.

The Ritual Bond is bugged, clicking on the incense doesn't do anything.

Grimclaw's Return not obtainable.
An Ocean Not So Deep isn't working, Gary is spawned, but speaking to him does nothing. Killing the Greymists does nothing. Btw. the whole Greymists species is suffering from cloning. There are far too many of them arround.

One Bitter Wish can't be obtanied, npc is spawned but doesn't give you the quest.
Fixed in V4.

War Supplies is bugged. Using the torch only sets the character on fire and the crates aren't interactable.

Timely Arrival just auto completes itself.
